Das Spiel versetzt den Spieler in eine post-apokalyptische Welt. Vor 75 Jahre ist ein Komet auf die Erde gestürzt und hat diese stark verwüstet . Nun regiert Anarchie diese Welt. Das Spiel soll eine Mischung aus Renn- und Actionspiel sowie First-Person Shooter werden.

Rage besitzt ein Handwerkssystem, mit dem sich unter anderem Autobomben und Waffengeschütze bauen lassen, sofern man denn über die nötigen Materialien verfügt. Die Fahrzeuge selbst können mit Updates ausgestattet werden, wie etwa Schilde, Reifen-Messer, Bomben oder Magnete. Eine ganz spezielle Waffe wird der Wingstick-Boomerang, der scheinbar im Ego-Shooter-Modus benutzt wird. Wurde man getroffen, so kann die Gesundheit selbständig generiert werden.

Wer eine Armbrust besitzt, kann zudem einen bestimmten Bolzen benutzen, mit dem sich getroffene Gegner für eine kurze Zeit kontrollieren lassen. Danach explodiert der Bolzen im Gegner. In Rage wird es auch eine Art TV-Sendung namens Mutant Bash TV geben, in der man Geldpreise gewinnen kann.

Mit dem Ego-Shooter Rage geht das Entwickler-Studio id Software neue Wege. So ergänzt der Doom-Erfinder die Ballereinlagen um eine vergleichsweise offene Spielwelt, kleine Rollenspielelemente, Quests und vor allem Auto-Rennen (tiels mit Waffen). Der Spieler erwacht in Rage 100 Jahre nach der Apokalypse in einer Endzeitwelt und zieht in den Kampf gegen Banditen, Mutanten und die Regierung. Dabei holen wir uns die Aufträge wie in einem Rollenspiel ab und werden für erfüllte Missionen mit neuen Waffen, Upgrades und Geld belohnt - müssen aber auch viel hin- und herrennen oder fahren. Rage ist das erste Spiel mit id Softwares hauseigener Grafikengine id Tech 5. Der Multiplayer-Modus beschränkt sich auf Vier-Mann-Autorennen und neun Koop-Missionen.
